The certificate is issued in addition to the death certificate. It serves as formal proof of the deceased’s inmate status at the time of death, for use in legal, administrative, or insurance-related procedures.

Follow these simple steps to help a user apply for Confirmation of Inmate's Death in a Correctional Facility
An application number (C2...) will then be sent to the applicant so they can track their application status.
